Physical Therapy & Athletic Enhancement
Welcome to For Life Therapy!
At For Life Therapy, we work with you on-on-one with your personalized treatment plan. Most appointments start on the top of the hour and last about 1 hour.
First Appointment: The Evaluation
Please arrive 20 minutes prior to your first scheduled appointment and bring the following:

Health Insurance Card(s)

Referral Forms from your Doctor (if any)

Medical Notes or Reports from your Doctor (if any)
New patients may request a copy of the the New Patient Intake Packet to be emailed. Please print, complete, and bring to your first appointment.
Follow Up Appointments
Please arrive 10 minutes prior to your scheduled time as we are often able to start a few minutes early. Please:

Bring a form of payment for your copay (if any)

Wear appropriate clothing or shoes to treat the area we are working on. We do have shorts, tanks, and gowns available.

Arrive to all scheduled appointments, even if you are feeling better or worse. Your Physical Therapist will adapt your session accordingly and make recommendations.
South Dakota is a "Direct Access" state for Physical Therapy and most insurances, meaning that you do NOT need an "order" or "referral" from your doctor (though we will gladly communicate with your doctor if you choose.) If your insurance requires a referral for Physical Therapy, it is your responsibility to obtain one from your doctor. Please call us to see if your insurance requires a referral.
Our Doctors of Physical Therapy are In-Network with most health insurance companies. We will verify your eligibility and benefits at your first appointment and let you know expected costs for Physical Therapy. It is ultimately your responsibility to know your insurance plan and coverage details. Please call us to confirm your specific insurance carrier.
Prior Authorization
Some insurance plans require prior authorization from the insurance carrier. We will verify your benefits at your first appointment and apply for authorization on your behalf. Workers' Compensation insurance does require authorization before your first appointment can be scheduled.
Patient Responsibilities
Notify For Life Therapy immediately:

Of any changes in insurance, contact, or mailing information.

If you have gone to the chiropractor on the same day as your Physical Therapy appointment.

If you are on any home health services, regardless of the service. All services, including Physical Therapy, must be provided by the home health care team.

If you need to cancel or reschedule an appointment.
Your health insurance coverage is a contract between you and your insurance carrier: copays, co-insurance, deductible, and out of pocket expenses are your responsibility to pay to For Life Therapy, Inc at the time of service. It is ultimately your responsibility to know your insurance plan's coverage and benefits. Account balances are due 30 days from the date listed on the statement sent to you. Payment Plans may be arranged with For Life Therapy staff, not assumed. Please call with any questions. Thank you.
