Welcome to For Life Therapy! 

For your convenience, new patients may print the New Patient Intake Packet, from the link below, to complete and bring to your first appointment.

 

Please arrive 15 minutes prior to your scheduled appointment and bring the following:

 

-Health Insurance Card(s)*

-Completed Patient Forms (below, optional)

-Referral Forms from your Doctor (if any)**

-Medical Notes or Reports from your Doctor (if any)
 

* Our Doctors of Physical Therapy are In-Network with most health insurance companies. Feel free to call us and ask about your specific insurance carrier.

** South Dakota is a "Direct Access" state for Physical Therapy and most insurances, meaning that you do NOT need an "order" or "referral" from your doctor (though we will gladly communicate with your doctor if you choose.)

Only a couple insurances in our area may require a doctor's referral or a preauthorization from the insurance carrier.

Call us first to determine the requirements of your specific insurance. Our staff is happy to help you navigate these requirements.
