We will verify your insurance plan's eligibility and benefits at your first visit and inform you of the expected cost for your physical therapy treatment. It is your responsibility to notify us of any changes to your eligibility and coverage benefits. We are dedicated to providing the highest quality, cost-effective physical therapy to you.
Understanding Your Coverage
It is ultimately your responsibility to know and understand your insurance plan's coverage and benefits.
Your premium is the monthly payment to the insurance company. If you don’t pay the monthly premium, you will lose your health insurance plan.

Your deductible is the money you will need to pay for care before the insurance company starts to pay its part.
Your out-of-pocket-max (maximum) is the most you will pay in 1 year for covered health services in addition to the premium. Copays or co-insurance typically help meet your out-of-pocket maximum.
Copay is the fixed amount that you pay for a service. For most
plans, if you have a copay for a service, you will not need to meet
the deductible first.
Co-insurance is the percentage that you pay for a service. This
means that the insurance company will pay a portion and you will
owe a portion. Each visit maybe a different amount and
co-insurance typically follows the deductible.
The term “covered” means that your insurance plan/policy is active and allows a service/item to be billed to them. Your insurance decides how the covered service will then be paid for, whether that is paid by you (deductible, copay, etc.) or by your insurance. It does not necessarily mean that the service/item is free to you.
